I retired from service as SS/MTNC /CHTS on[protected]. I have given my undertaking & option through the pension book on[protected]. Even after the completion of 4 months from my retirement I have not received this Fixed Medical allowance from the bank. When enquired it is informed that this information was not been recorded in the PPO. Hence I humbly request you to kindly instruct SBI/ Muvattupuzha Branch for the sanctioning of Rs 500/- as FMA per month and also with the arrears from[protected] onwards.
