SRL Diagnostics — Bill / Invoice not received for Corona tests | |
Girish M Agarwal had undergone Covid tests on 7-8-20 and 13-8-20 Laxmi Agarwal had undergone Covid tests on 7-8-20 and 15-8-20 I have been following up with SRL for bill/invoice for over 5 months, so i can submit to Insurance company and claim reimbursement. I have also provided following information requested by SRL for reference numbers i have for those tests. Same are below for your reference. Laxmi G Agarwal - 7-8-20 - PERM ID LAXMF[protected] Girish Agarwal - 7-8-20 - PERM ID GIRIM[protected] Laxmi Agarwal[protected] Receipt No ICR[protected] Girish Makhanlal Agarwal[protected] Receipt No ICR[protected] I also discussed over phone with their person who promised to send invoices but for a while his number is also not connecting. The report on letterhead does not carry address of lab. Please let me know if you need any more information. Request to please support with obtaining invoices from SRL.
