SRL Diagnostics — Bill / Invoice not received for Corona tests

Girish M Agarwal had undergone Covid tests on 7-8-20 and 13-8-20
Laxmi Agarwal had undergone Covid tests on 7-8-20 and 15-8-20

I have been following up with SRL for bill/invoice for over 5 months, so i can submit to Insurance company and claim reimbursement.

I have also provided following information requested by SRL for reference numbers i have for those tests. Same are below for your reference.

I also discussed over phone with their person who promised to send invoices but for a while his number is also not connecting.

The report on letterhead does not carry address of lab.

Request to please support with obtaining invoices from SRL.
