SRL Diagnostics — Technical efficiencies

Sir,
This is to bring in your notice that i Munear Ahmad Khanday visited SRL Diagnostic Center Gujar Nagar Jammu and submitted blood samples for various tests, such as Basic lipid profile, TSH & Uric acid serum on 13.022016 and i was accordingly asked to get the reports on 13.02.2016 in the evening hours.
after going through the tests reports i found out some of the mistakes made by the laboratory which is as under:
1. Name has been addressed as Mumeen Ahmad instead of Munear Ahmad.
2. Sex Gender has been placed as female while i am Male .
As such i am not satisfied and am in doubt, so it is requested that the same tests may be re-checked and if necessary i am ready to re-submit my blood sample.
early action in the matter will be highly appreciated.
