State Bank Of India — Account opening in process from last 3 months

Hi, On 17 november, 2014, i was submitted two forms of my children for open an account, I was waiting for last 3 months to open an account, on 02/02/2015, i visited to the bank again and they told me u r form is lossed by me, how can they do these. another thing is that my husband submitted his form last months, they always told, pls come after 8 days, 9 days . but nobody is replying. All that happened in SBI ( Gateway of India) Branch.
