State Bank Of India [SBI] — closing my csp account in sbi

Address:Baladas ghat west of anjaanpeer chowk, Vaishali, Bihar, 844101

I have opened my CSP account in SBI of branch Ramashisj Chowk, Hajipur, Bihar last year.Now, I want to open a natural saving account in SBI branch .but I cannot open my account until I close csp account. I have lost my account card and my atm . I have no data about my account details to close my CSP account.please help me so that I can close my CSP account .
