Sub: delay in pension for spouse Ref: application given to state bank of india chinchwad, pune dated 25/10/2019 Dear sir/madam, This is bring to your notice that my husband, mr. Jayant s vedpathak has expired on 18th aug 2019, (Necessary death certificate is enclosed herewith along with copy of ppo). As per procedure, i had approached the state bank of india (Sbi) branch – hdfc complex, old bombay pune rd. Chinchwad pune – 411019 (Mh), and submitted necessary application as per given format, and submitted the same with necessary support documents dated 25/10/2019. Please note that, my family pension has still not started till date. (From sep 2019 up to 2020). As i am facing financial headships, i request you to kindly look into the matter personally and start my pension for spouse (Smt. Vasanti jayant vedpathak) age 73, as soon as possible and do the needful.
