State Bank Of India [SBI] — issue of two atm cards

Address:Rewari, Haryana, 123401

I have applied for only one atm card but two atm cards have been issued and charges are deducted from my bank account which is not fair as i didn't applied for the second card. Kindly take the necessary action and do the needful. It shall be highly appreciable at this end.
