State Bank Of India [SBI] — non issue of atm card to sr. citizen lady

I have an sb a/c in sbi chakeri kanpur, a/c no. [protected]. I have visited thrice to my branch for issue of atm card and given application every time. But till now no atm card is issued to me. I am 65yrs old sr. Citizen lady, i can't visit branch every time for cash withdrawls, hence i require an atm card most urgently.

Because of i lost my mobile with sim card.so i can't able to received any sms about my policy status from sbi life insurance.
