State Bank Of India [SBI] — replacement debit card with incorrect details

My debit card was not working at any atms and hence as per the branch manager's advice i had applied for replacement of my expired debit card on 19th november 2017. The card did not arrive even after the 10 days duration. So had to visit the branch again on 30th november 2017 and the employee who processed the request told that the request got rejected and he told me some vague reason for not informing me about the rejection. He made me submit the another request form again and confirmed that i would get the card within next 10 days. After few days i received the new replacement debit card, but the name printed on my card is not correct. I am a working woman and hence find it very difficult to visit the branch multiple times to correct these errors made by bank employee. This irresponsible attitude of sbi branch employee has been very disappointing and i have to say that this is not a customer friendly bank. I am not able to make any transactions via atm due to this issue for about 2 months now.
