I had requested sbi branch manager, sbi modal town ghaziabad to exclude mediac allowances of rs 1000/= from my pension so as to aplly for cghd (New) wide my application dated 23.1.2019 but i have not got any response from them and so i could noy apply for new cghs cadrs.

It is requested that medical allowance may kindly be deducted/ excluded from my pension immediately and i may be informed accordingly on my address - iiird h 141, nehru nagar rakesh marg ghziabad 201001 up so that i may apply for new cghs plastic card to avail medical facilities. My ppo no.[protected] and bank pension account number -[protected]and mobile no - [protected].

On 11 mar 2019 I used Bhavnagar district bank arm for withdrawal of Rs.8000 but due to some technical issue cash is not deapense but 8000 debited from my saving account no [protected], SBI Jesar branch. Till date debited 8000 Rs is not deposit back to my sbi saving account
[protected]. Account holder Name Sureshbhai Bhikhabhai Ulava.
