Dear Sir, I had paid renewal amount of RS. 3539/- towards the renewal of my mediclaim policy No. [protected]P[protected] during August 2015. I had also received renewal notice form United India Insurance Co.Ltd., . Policy expiry date was 18.08.2015. Till date I have not received any documents for renewal of policy from you. My syndicate bank branch is in Vijayanagar (BKA[protected]).
