T24 Mobile — sim card lost urgently need to block

I have lost my t24 sim card on 10 dec 2018. So urgently need to block it. And kindly say what to do to replace my sim. The phone was stolen near my home and it appears that i would not get the phone back. The number is [protected] and this sim and taken under the name of dibyendu maitra. Kindly look into this matter and help me as fast as possible.
