Sir we have not able to receve maturity amount of the bond membership id [protected] the maturity completed on 4-9-2014. And by mistake the name of child and dob is not right on bond, and but the name of grandfather nd father is correct on the bond please help me for resolved the problem my mobile no. Is [protected]. We leave in kishangarh and the candidate is now get marriaged

Unit Trust of India Mutual Fund [UTIMF] Customer Care's response Units under the RUP 94 (II) certificate [protected] have been converted into ARS bonds vide bond certificate 33135106 for 30 bonds in the name of KUM KAJAL under Id no.[protected]. The said bond certificate was dispatched to unit holder's registered address, however the same is returned to us as undelivered by the postal authorities.

In order to relese the maturity payment and for name correction kindly submit the below mentioned documents:

1. Submit original Affidavit has to be executed by Unit Holder on non-judicial stamp paper value Rs.20 duly attested by Notary Public stating that old name and new name is of one and the same person .
2. Kindly get main holder signature attested by the Bank Manager under his/her official seal mentioning name, Emp. code, Designation, where the account is maintained.
3. Kindly submit the self attested copy of residence proof i.e. voter card / ration card / electricity bill and photo Id proof i.e. PAN card/Driving License/Passport.
4. Kindly provide the attested (Bank manager/Gazetted officer/Notary) copy of birth certificate.
5. Submit the original RUP 94 (II) certificate or any other investment proo[censored]nder the captioned investment.
6. Copy of cancelled cheque leaf or bank statement.
