United Healthcare — claim not reimbursed and terrific harassment

This company has harassed me to the heights .In the month of october my father was hospitalized whose medical expenses came around 180000.I submitted the documents for claim in the moth of November.The premium amount was deducted from my salary immediately .
I submitted my initial set of document in Dec .they asked for more documents.Initially i already clarified via mail that i work in bangalore location and the hospital is situated in kolkata .So whatever document is required let that be known to me asap .

But they were quite late in sending me back the discharge summary due to which the second set of document reached late to them
they received all the documents from my end by 9th February 2018 .
i still dont have any status .Every time i call them its like I am entering into a discussion which has no outcome .

they are so careless that they don't even check the documents sent to them and keep asking for the same documents again and again.
And their best excuse is "your phone was not reachable "
i am seriously harassed by this company and i really want some action to be taken against them .
Even after repetitively warning them that i will lodge a complaint against you in consumer forum they don't care .
I want the claimed amount back .That's my right .else if they still cant proceed with it I want my premium amount that i paid.
