Airtel — SIM LOST I NEED TO BLOCK

I lost my phone yesterday[protected] from bangalore near majestic evening between 7-8 clock, i want to block my sim when i called airtel cutomer care they are telling my addreess and name is wrong, but i took SIM in my id only in 2010 june, and am using that last 5 years, phone is still ringing . & i want to block it.
