Date: April 07, 2016 To, The Concerned Official Reserve Bank of India 6, Sansad Marg, New Delhi-110001 Dear Sir, This is to inform you that I Sunaina Dubey (Axis Bank Account Holder- A/c No- [protected] & Customer ID[protected] hereby wish to draw your kind attention towards the fraud happened with me. I Sunaina Dubey on dated 20/02/2016 went to Canara Bank ATM (ATM ID[protected] at Naraina Vihar, New Delhi to withdraw Rs. 4000/- (Four Thousand Only) from my Axis Bank Account (details mentioned above) however the Cash transaction was not successful due to some failure at the ATM but during the said duration I have received the SMS from Axis Bank stating that the transaction was successful and neither I have received the transaction slip from the ATM. Furthermore to cross check I once again did a transaction of Rs. 200/- and same thing happened (Transaction was unsuccessful however the SMS for successful transaction received). PLEASE FIND ATTACH THE TRANSACTION SLIP FOR RS. 200/- for your reference purpose I have escalated this matter to Axis Bank at their respective Customer Care numbers however no satisfactory response from their end is received till yet. I have requested them to kindly check the CCTV footage of Canara Bank ATM but my request and repeated approach went in vein everytime. Since the incident date I have been continually approaching them to solve this issue but the kind of treatment I have received from Axis Bank has harassed me to the core. Sir please note that I been a middle class working women have the important of every single amount that I have earned. This Rs. 4000/- + Rs. 200/- means a lot to me and loosing it without at my any fault is a pure state of distress.
