Patient name: neha jaiswal gupta Policy holder: bishwajit sinha Policy no.: og[protected]-00001953 Original documents, kyc and cancelled cheque sent through dtdc courier on 17/6/2019 Deficient raised by bajaj on neft details and pending documents Response sent to bajaj on 2nd july regarding neft details & previous consultation papers. Bajaj allianz wrongly put insurer name as bishwajit instead of bishwajit sinha Name correction done and details sent on 22nd july, 2019. Document recovery team constantly asked for neft details of bishwajit instead of bishwajit sinha wherein i have submitted all the valid documents against that.
