Sir, i would like to inform you that i had visited your corporate office located at s. P. Verma road, patna regarding cancellation of bajaj insurance policy on 15.05.2018. They had advised me to fill up cancellation form if you are not interested in the policy. Accordingly i filled up the cancellation form with a request to cancel the policy because the policy was given to me without my consent. After lapse of more than 18 days, today i. E. On 05.06.2018 i again received a message from bajaj alianz life insurance on my mobile no. [protected] that "your bajaj allianz policy number is og[protected]-[protected]Mediclaim insurance policy - (For bfl customers). Your mailing address is flat no. 301 e block jagmano shree garden apartment ved nagar rukunpura behind ssb office patna, bihar -800014". I am astonished as to why this has happened when i had already given my request for its cancellation well before in the said corporate office.
Therefore, i would like to surrender my bajaj finserv emi card bearing no. [protected]. I request you to please deactivate my bajaj finserv emi card bearing no. [protected] and mediclaim policy also. I do not require any such card. Thanking you, subodh kumar
