Delay of Renewal of passport for an year

Respected Sir ,
My application for renewal of passport -- file number -- F/037985/10 has delayed for a long time for about an year . This is the second time I am renewing my passport a sthe first time the police did not get notified and the second time , the enquiry report has been sent to RPO on 7/6/2011 .

I humbly request you to look into this matter and help me out...

Renewal passport not received

Dear Sir,
My passport was issued from Delhi I hope so in approx 1988, but at the time of renewal I was in Lucknow, for renewal I submitted my passport in LUCKNOW PASSPORT OFFICE, I enquired many times personally about renewal but they could not tell me the exact position,due to my transferable job then I transferred to Bareilly and after that now I living in Raipur(Chattisgarh)since 2003. But now my all details lost during shifting...
