Reliance General Insurance Co. Ltd. — hospitalization bill of rs.27098/-(bill amount rs.23098/- plus

Sir ,
I was admitted in Ayushman Hospital &Trauma centre,hero Honda Chowk,NH-8,Gurgoan(Haryana) on 02/02/2010 at time 1.15P.M. due to problem was diagnosised Hepatitis.I contacted to your TPA Medi Assist India Pvt.Ltd. on his toll free no.[protected] so many times but TPA has not picked my phone.
I made a complain to Relaince General Insurance Co.Ltd. and got registered no. 947725. The Relaince General Insurance Co.Ltd. has given me Delhi address of Medi Assist India TPA Pvt. Ltd.F-2,Kailash Plaza ,H-252,Sant Nagar,New Delhi-110065 Ph.[protected]/11.I contacted TPA Delhi office through phones no. Ph.[protected]/11 and office person told me to deposit your claim with origional Claim Form Filled with Origional Hospitalization Documents(Admission Sheet, Discharge
Card and Bills of Ayushman Hospital &Trauma centre,hero Honda Chowk,NH-8,Gurgoan(Haryana)).I deposited Origional Hospitalization Documents(Admission Sheet, DischargeCard and Bills of Ayushman Hospital &Trauma centre,hero Honda Chowk,NH-8,Gurgoan(Haryana) along with Claim Form Filled to receiving section of Medi Assist India TPA Pvt. Ltd.F-2,Kailash Plaza ,H-252,Sant Nagar,New Delhi-110065 on 11/02/2010 wide register no.21 .The scan copy of receipt given by receiving section of Medi Assist India TPA Pvt. Ltd.F-2,Kailash Plaza ,H-252,Sant Nagar,New Delhi-110065 is herewith attached.
Therefore, you are requested to sent the chaque of Rs. 27098/-( Hospital bill of Rs.23098/- and with four days bed charges @1000/- per day or Rs.4000/- bed charges)
as early as possible.
