my name is Gulshan Ara my A/C no. [protected] & my ATM card no.[protected]. i changed my pin no on date 29/12/09. after some days i forget my pin no & my ATM card is blocked.

My atm debit card has been retained by Maharashtra co-op bank as i have not collected the card from the machine. As the card has been destroyed i want to block the card and apply for a new atm card .I request u to block the card .

As i have lost my ATM card yesterday night near santoshi mandir ,rasulgarh,I want to block my ATM card bearing number[protected].It was related to my Industrial estate,palasuni branch SBI joint account(Dr gouri ku padhy and mrs mahalakshmi sahu) having account number [protected].
