Sir,
Kindly refer my request dated 16.10.2007 at UTI, Hyderabad office, regarding non receipt of maturity amount pertaining to my ULIP Membership: No. Ul[protected]. I request that:

1. Since the maturity amount o[censored]nits (2523.091) pertaining to my above said policy is with UTI even as on date(08.09.2008).

2. Since I have not recieved any amount till date I request you to close the a/c and pay the amount immediately to the adress given below

Kindly expedite the matter.

I had purchased ULIP during 1990 vide membership no. UL[protected] and paid all the installments. The above scheme has already matured and accoridingly I requested for payment of maturity proceeds. However, to my surprise, I was informed by UTI that as per their records i have not paid some of the installments. In this connection I would also like to mention that I was never informed of non payment of any installments even after receipt of subsequent installments by UTI. The proof of payment of 10th installment, which was shown as not received by UTI, has also been furnished by me to Mr. Suman Mukherjee, Manager, UTI, Kolkata vide my letter dated 19/03/2008. It is not understood as to why the UTI has not informed me about non receipt of installment while accepting subequent installments. On receipt of my above proof, Mr Suman Mukherjee has addressed to The Chief Manager, UTI, Bhubaneswar vide his letter No. UTIMF/KOL UFC/IRC/MISC/2007-08 dated 28/03/2008 to reconcile the record and sed remitence to him. No further response has been received till date in spite of several reminders. In this connection I would also like to inform that I am unable to contact any one in your office as there is no resonse on any telephones.
I, thereefore, once again request you to kindly look into the matter and arrange to pay the entire procce at my following address.

Dear Sir,
I am surprised to hear that out of 10 I have paid only 6 installments.
In this regard, I bring to your kind notice that such letter was also received earlier at the time of maturity of my other ULIP No UL[protected] , installment amount Rs. 4500/ annum. At that time I have submitted the documents to prove that I have paid all the installments regularly to the Registrar, UTI mutual Fund, UTI Tower, GN Block, Bandra – Kurla Complex. . Finally, based on such documents full amount was paid to me. In that letter I have enclosed the receipts even for the present ULIP too.
However, I am again attaching the scanned copy of the receipts pertaining to the present ULIP (Rs. 3000 / annum) for your reference and record.
Further as far address is concerned, the documents were also sent. Subsequently I started getting the notice on the present address. However, as required, Pan card copy, cancelled cheque (original) address proof, etc. have already submitted to your branch office namely Karvy UTI NHP on 29/03/2010 , receipt of your office D54004, Nehru Place, Delhi.
Hope the attached documents and the documents submitted to your office will clarify your all doubts.
Kindly arrange to send the payment at the earliest.
