Claim No RR-HS[protected] Claim Status Query Request Claim Type Reimbursment Claim Intimation Date 06/07/2015 Proposer Name PURUSHOTAM AVULA HDFC ERGO ID ER[protected]-02M I have followed all the process of insurance claim procedure and now they say that we haven't received the cause and treatment for the same. Which is already written by doctor on back of discharge form and when we clarified with him he even shown it to us and said that is what all you required for claim procedure. when I spoke to HDFC representative they said they will check the same and will get back to me, but there is no reply from them. Please help me on this. If you guys don't have issue providing insurance with same ID and now for reimbursement they have issue with my name in account, which is same in my ID used for insurance.
