I would like to bring to your attention that I have strong evidence of fraudulent and incomplete information of Form 60 and other initial documents of my wife's account with HDFC Life. Attached are the documents and email communication. The account was opened in 2016 when my wife was residing in Canada. She never visited India in 2016. She last visited India in 2007. Yet all information was as if her location of signing was in Mumbai. Plus as a non resident, HDFC Life claimed that she was a resident of India. Form 60 to my knowledge as per law is a requirement to be only filled and signed and dated by the potential policy holder prior to acceptance by HDFC Life. This was not the case with HDFC Life.
To my knowledge HDFC Life was using fraudulent Life insurance account in the BSE stock exchange.

I received your documents. After reviewing your attached documents, I declare that the policy documents HDFC Life created for policy #1842269 was invalid, fraudulently created with false information.
The requirement as per "Proposal form for single Life" stated that the entire form also known as Form 60 as per HDFC Life was to be filled complete by the policy holder. This was not the case.
Also, the form required a witness and signature. Who was the witness? I did not see the policy holder's signature and the place and date where she signed her Form 60. Why was HDFC Life careless in their undertaking? Where is the accountability?
In PDF # 2344_[protected] under "Declaration in Lieu of Pan Card" shows that the place that Winifred D'Souza supposedly signed an undated form, was in Mumbai. The form without a date has no validity. Also, this document you produced was falsely noted as signed in Mumbai. Allow me to declare that Winifred D'Souza as once holder of HDFC policy #18482269 had never visited India in 2016. This document you produced stated false information.
In the "Customer Consent Document", Winifred D'Souza was never a tax resident of India in 2016. Also, for more than 30 years prior to 2016 she was not a resident of India. So why was Block "yes" been checked? No attested or verifiable photograph was included? This is a Regulatory requirement in 5.3 in RBI Master Circulars.
What analysis did Varsha Satam do by using the HDFC suitability Matrix? Show me the contents of that analysis?
And to top it all, Winifred D'Souza's signature was again supposedly signed in Mumbai dated 28th April 2016. This is a false statement.
Varsha Satam, Manager, on behalf of HDFC Life declared that she carefully read and understood the declaration and terms of the above policy This action, criminal in nature by HDFC LIFE personnel in approving and supporting false information is unacceptable and due process of the law should be applied.
Trading in invalid policy funds in the stock market are a crime. Policy #18482269 was created intentionally to benefit HDFC Life and personnel.
Incomplete and false statement policy documents by HDFC Life and personnel of policy # 18482269 not only invalidates the policy but is also subject to prosecution and financial charges. Since none of the documents including "Form 60" presented showed that the waiving of pan card documents was complete and/or with false statements, therefore intentionally opening of above policy and operation was invalid.
Allow me to quote some of the legal terminology. "Consequences of wrong declaration. Making any false statement in Form 60 may attract these consequences: If the amount involved is less than Rs. 25 lakhs: imprisonment from 3 months to 2 years along with a fine can be imposed. If the amount involved is greater than Rs.25 Lakhs: imprisonment from 6 months to seven years along with a fine can be imposed.
I did not surrender Rs. 50 Lakhs of my funds for charity to HDFC Life and received zero returns. This policy that was offered was to increase HDFC Life sales figures by enticing, defrauding and swindling clients. The policy cannot be in "withdrawn status" as it was invalid from the start. What did HDFC Life team security investigate? They appear to be corrupt or support the corruption process.
If HDFC Life ignores this email and chose not to respond to a satisfactory explanation within 72 hours of receiving this email, legal process of reportable action will be taken.

That attached policy document has no validity. Surely, opening an account with a Life insurance company involves signatures of client as well as Life insurance personnel. Please send me the right policy document with signatures and pan card information including the completed Form 60 which was mandatory in 2016.

I had spent over 3000 hours of my time in researching, emailing in getting HDFC Life and bank to acknowledge their corruption tactics and they keep deflecting it. This has caused me a lot of stress, anxiety, mental and physical health issues, time lost in carrying out normal life and monetary loss.
