Dear Sir, Sub: Claim No GEN[protected] of Mulchand Singh Policy No. 4065/ICICI-HSP/1389086/00 With reference to the above Claim, I hereby inform you that my husband Mulchand Singh was expired on 08/12/2012 at Jaipur and he had ICICI HEALTH policy No. 4065/ICICI-HSP/1389086/00 and valid till April-2013. And I have filed this claim in Jan-2013 with all necessary documents i.e Police FIR, Post Martem Report, Death Certificate, etc but till date this claim is not setteled. All these documents dispatched to your Hydrabad Gadchibowli office by Speed Post EMS. After that ICICI Lombard send me letter asked all the documents attested with concern authority. Again I get attested all the documents with Police Station and Hospital from Jaipur and despatched the same at same address of Hydrabad office. And in between they given me notice for Closure of Claim. But I inform them to hold the claim as i am in process to get the FSL Report. After all these they send me letter for documents required -FSL Report. We get FSL report at least after one Year and submitted the same to ICICI Lombard. And ICICI LOMBARD not giving reply properly. When we call in customer care some time they inform me that claim is under process and you will get reply within one week after few days when we call them they inform me requirement of documents. And in March-15 also they send me reminder of closure notice. And now they send me letter documents required Revised Post Martem Report. This health policy is against ICICI Home Loan taken by my husband and they continuously demanding for payments and given notice for auction and this claim amount Approx Rs 355000 (Three Lakks Fifty Five Thouand Only). Which is equivalent to pending home loan amount at that time. Enclosing all above documents with this letter. We would like to inform you that your request for claim is not processed as we require below documents:
· Cancelled cheque with payee name printed on it or NEFT mandate verified by bank for the payment account details of the nominee (LAXMI MULCHAND SINGH)
· As the payment amount increases 1 Lakh, please provide two passport size photos of the nominee (LAXMI MULCHAND SINGH) with address and ID proof
Post receipt of the required document claim can be further processed
