I have purchased a Bhavishya Arogya Policy No 1399 on 08.03.2004 in the name of Prabhudas Somaji Tembhurnikar. Sum insured for life time 180000/- sum insured per illness 72000/-. As per policy document policy commences wef 08.03.2015. This policy was issued by your Supela Bhilai office. I am suffering from blood pressure, diabities and asthama. I am under treatment of doctor for these illmess. Please guide me how I can claim my medical expenses biils. Please arrange to provide me specimen copy of claims from so that I can claim my expenses and benifitted by the scheme. my address is given hereinbelow.
