[Resolved]  Oriental Insurance — Correction of Names in PNB-Oriental Royal Mediclaim Policy scheme

In my Policy No. 422490/48/2013/53, my name was wrongly entered as Dr. B. JAYASHAKANKARA and my wife's name as B. H. GEETHA instead of Dr. B. JAYASHANKARA and G. H. GEETHA respectively. Hence, I hereby request you kindly to incorporate the above said corrections and oblige.
