Paytm — Amount deducted but recharge failed for mobile no. [protected]

I tried to recharge my airtel prepaid mobile no [protected] throgh paytm on1 jun 2016. However, it did not recharged and the amount of rs 250 deducted, which yet not recieved in my bank a/c or my paytm wallet after lapse of 13 days. The details was as under:-
