Reliance Communications — PF Transfer Required

I was employed in Reliance in 2 different periods.

1. Reliance Telecom Ltd. Bhopal (MP)
period- 7th April'06 to 1st May'08

PF No. not mentioned in payslip.

2. Reliance Infocom Ltd. in Ajmer, Rajasthan
Period - 1st Dec'14 to 31 Mar'15
PF No. MH/46459/20052831

port 31st Mar'15, the company name was changed to Reliance Comm. Infra Ltd.
period- 1 Apr'15 to 30th Oct'15

PF no. MH/46711/20052831

the PF was internally transferred from Reliance Infocom Ltd. to Reliance Comm. Infra Ltd. for which I had filled Form 13- Transfer claim form.
And so i expect my entire PF amount from 1st Dec'14 to 30th Oct'15 is under PF No. MH/46711/20052831 where it was transferred.

Now, i need to transfer my entire PF money (Both employer & employee contribution) to my current organization, details of which are as below-

so, I now request for transfer of my entire amount of both the stints in Reliance, from 7th April'06 to 1st May'08 and then 1st Dec'14 to 30th Oct'15 to my current organization, for which is have provided entire details that need to be fill in Form 13.

pls help me on the same as i have been trying since many months, contacted many people, but not able to find out a way for the transfer, as the Reliance Communication offices are already wind up and so i don't have any wayout.
