State Bank Of India [SBI] — family pension not received yet after 08 years.

I murti devi, my husband died in 2010. From last 08 years, i am chasing for family pension. At this point pcda, allahabad ask me to provide a “pda certificate mentioning the exact date, month and year of the payment of the pension made to the last pensioner indicating name of the pensioner and ppo no. "c/engr/840/1995" against which the payment of the pension was made” by the bank. My husband’s account was in state bank of india (Earlier state bank of patiala). Account details of my husband are given below:-
