State Bank Of India [SBI] — Money inserted to cdm but not credited to my account

Dear sir,
I smt. Ratna bhandari address village and post office garalgacha, district hooghly, police station chanditala, near surobala vidyamondir today dated 26/03/2016 are attempted to deposit rs. 35000 (Thirty five thousand) in two stages.
But first transaction of rs. 20000(Twenty thousand)was successful but in second transaction which is rs. 15000(Fifteen thousand)inserted to cdm but not credited to my account nor returned .
Please note sometime worn note came from atm and sometime this type of problem.
