To, The branch manager State bank of india Upper bazaar, chandi Subject: charge back request for the refund of amount rs. 10499/- Dear sir, I, manish kumar, an account holder in sbi, upper bazaar branch, wish to draw your attention toward the debit of an amount of rs. 10499/- on 19st jan 2019 as per the pass book details. It can be seen that there is no transaction done by me on the date mentioned. An application regarding the same has already been submitted to this branch on date: 30/01/2019; but there has been no positive response till now. Thereafter, again i submitted the application to the branch. Besides, i was asked to fill up and submit a complaint form, but the form was avoided to accept by the same banker. I am out of town and it is very difficult for me to manage with a handful of money over here. Therefore, i request you to kindly look into the matter and initiate the refund procedure as soon as possible. I am very sorry to say that in case no necessary step is taken this time, i would be compelled to take some major actions, as i cannot afford to bother my family any more to come to the bank again and again for the same purpose.
