Syndarogya Group Mediclaim — no communication from synd arogya health insurance policy taken at new nallakunta syndicate bank

Hi there,

I've applied for synd arogya health insurance policy at new nallakunta syndicate bank branch on 23/10/2018 with dd no - 430553 amounted rs.7683/-. Though i've submitted my phone number, email and address details accurately while filling up the from, till now there is no intimation regarding my policy number from neither bank nor from united india insurance company.
I've contacted concerned agent mr. Dharmarao, number of times (20 to 30 times) and also bank authorities, and every time, i am receiving a reckless answer from mr. Dharmarao, saying that he is busy and details will be sent by sms.
And now after almost 35 days, he is saying that, i've to go and collect the insurance document at the bank and the card will be delivered later. Though he claims that the policy document was not delivered in my address, i am sure, he did not send the document by post or courier. Since i am staying in the same address for last 20 years and it's my permanent address which is mentioned in my adhar card, there is no chance, the courier would be undelivered.

Is it not the responsibility of the insurance company to send the policy details and the health card within 15 days of payment of premium?
Who is responsible for this? Bank authorities / insurance company?
If the things are going with this pace, whom should i contact in case of any insurance claim? Bank / insurance company? How can i expect the claim on time from such irresponsible company?
Since, i am considering this very serious and hurting, i request serious action against the concerned persons responsible for this for causing inconvenience to me by not communicating the policy number, and not sending the policy document and health card within 15 days to me.

Res sir i am continued syndicate bank syndarogya family helth insurance in 2 years, for this time syndicate bank told after 1 april 2020 the policy not posible renewal, what shud i do plz hel and take action
