Dear Sir,
I worked for Wanbury Ltd, BSEL Tech Park, B-Wing, 10th
Floor, Sector-30A, Opp Vashi Railway Station, Vashi, Navi Mumbai-400705. I
joined the organisation on 28/01/2008 and resigned on 13/06/2011. I applied for
withdrawal of my P.F. amount in the month of November 2014 and sent duly filled
Form 19 and Form No 10-C along with cancelled cheque to Wanbury office.

The tablet FOLININE were prescribed on 14.08.10, the tablet strip was bought on 14/08/10 - my wife used the table for 4 days & on the fifth day when 9th Tablet from the strip was opened, we found it completely contaminated with fungus. We confirmed that the this was intact tablet and was not exposed to any moist surroundings. I clearly see a manufacturing faults OR poor adherence to GMPs in the plant.
