forgot irctc login user name & Password - IRCTC

I have forgot my IRCTC Login user name &Password.

my mobile no:[protected]

I have forgotten my user id and password, and am told anyway that the account for 'Kamlesh'
has been deactivated.
How do I reactivate the old id, or otherwise make a new id?
