Dear Sir/Madam,

Many time we will contact to the customer care regarding this claim, and all time they misguide me. continue one month they misguide us regarding claim. I had clarify all the details on mail but they could not guide any of the time.They try to west my time only and try to cover up this march claim only.

With reference to your request for providing doctor's prescription for hospital admission dated[protected], I would like to reiterate that the patient Mr. Subhas Chandra Ganguly was shifted from Relife Nursing Home to Institute of Neurosciences Kolkata (INK) on[protected]. The arrangement was made on emergency basis as patient needed specialised care and the kind of ailments he was having could be best handled in INK, being a reputed hospital for curing neurological disorders. We therefore made the necessary arrangements as soon as possible and got him transferred so that his condition could be improved on urgent basis. Also, Doctor's prescription dated[protected] was already attached and sent on mail dated[protected]. The perscription was issued by Dr. Kausik Datta, the doctor visiting the patient while he was in Relife Nursing Home.

Furthermore, all the required details have already been shared in the chain mail. Why is it that additional prescription for hospitalisation is required? Are the details already provided not enough? We have provided everything including prespcriptions and discharge certificates which should hold good for claiming the expenses. Please check and recheck all the details already provided and process the claim. I would be expecting no further queries in the process.

Kindly look into the matter on priority basis.

Dear Sir/Madam,

With regard to the above details, I have constantly followed up with the executives of Family Health Plan TPA for the medical expenses claim of my father, but the officials have not been responding well. Initially while receiving my claim requests, they asked me to mail all the documents and details and said they would look into the matter after that. I provided them with all the documents but after that they simply denied having asked for any mails. So how can I expect to receive the service when they keep on changing their versions? After doing so much of work in arranging and sending the requested documents, they are still refusing the claim requests. Also the FY is close to end which will make me unable to request for any claims after this. So how can I now get a solution? Kindly solicit my request to them and guide me on this.

This is to inform you that in spite of repeated followups with the FHPL TPA for the claim requests, they have again demanded the same set of documents and statements as asked earlier, image of their intimation letter attached. The queries that they have raised in the FHPL letter (which I received just yesterday after so many days of constant followups) have already been attended to, please refer mail screen shot of my Mar 15, 2019 conversation. Furthermore, I have already provided doctor's advice which contains the patient's condition and history. Please understand, how many times will I pursue for the same thing again and again? When will this end? Will I get a solution? Please help in mediating the issue as I have already clarified.

    I had submitted all the documents in the month of may 2018 but still this insurance company saying that there is not any medicliam under my name.
    I follow up on emalis but still they are not providing any inputs.
    What actual process they are following i don't understand.
    Is there any one who gets their claim by national insurance company ltd.
    Please let me know how can i get my claim.

      I have undergone surgery and submitted for claim but its been rejected giving wrong reasons. I had a severe head ache from past few months it was unbearable, i showed to many doctors but no medicines could cure my head ache later when i did scanning there was deviated nasal septum with spur development to the left which is the main reason for my head ache and other terrible problems. Later i took 2nd opinion from many other doctors because i personally very much scared of surgery. But there was no other go, i have to agree and under go surgery to get my head ache cured. Even though i had cashless in other hospitals but the estimation amount was quite more, i did paid the amount in 1 of the hospital where i got comparably less price and did surgery. But i see now i am being cheated by fhpl by giving wrong reasons and rejecteing my claim, i see there is no value for good deeds, with out doing proper investigation they are saying that its a cosmetic surgery. There is not even a 0.1% cosmetic treatment here, fhpl should do there work properly, go to hospital and check with my surgeon and show reports to other geniune doctors and i can also provide before and after pics of mine. I will never suggest fhpl to anyone. Kindly help

        I am Gagandeep Singh employed with HDFC Bank Ltd. & is enrolled with FHPL from last 5 years. I have submitted all original documents for reimbursement but they are marking unnecessary queries. I have provided them the details of admission & discharge on letterhead of the hospital along with stamp & signatures of the doctor & have even summarised everything on letterhead. I have even marked mail to them that please laise with the hospital authorities if any queries as guided by their customer care. But still they are marking query that discharge summary in not in proper format. I am being harassed now, simply useless services.
        My claim is 2069943/1 . I have bad experience and i recommend to all my 300 ++ associates not go with this company

          I am prasang khare from hdfc bank and my id is p11193 and i want to know about my claim limit as my mother is getting admitted/operated and i am been calling fhpl from last 5 hours but customer complaint number is not reachable. How can they entertain customers in need when their phone line doesnt work and their employees mobile number in indore gets responded with "busy in driving" or "on leave". This definetely seems to be a reason to go on social media and report the irresponsible behaviour. Hope somebody responsible will call and solve my query asap on [protected].

As per query requirement of discharged summary of hospital format but this is taluka level hospital, no available discharged summary. I have also submitted discharge summary on hospital letterhead with treating doctor signed and stamped. Please consider it and process further.

                Myself amitsingh bopche from gondia, maharashtra. I made health insurance policy of bharti axa on 17 august 2017. My policy no. Is ihs/q0103266/2c/08. I was admitted in hospital for the operation of cholecystitis from 20.06.18 to 26.06.18. I had send all the hospital, investigation and medical bills along with doctor prescriptions, discharge card and other required documents mentioned in the claim form through postal department by speed post (No. Em[protected]in & em[protected]in) on 11.07.2018. According to the report of postal department it is delivered on 17.07.2018 on the address of family health plan insurance tpa ltd, srinalaya - cyber spazio, suite # 101, 102, 109, 110, ground floor, road no. 2 banjara hills, hyderabad - 500034.
                After delivery i made so many calls to fhpl hyderabad for my reimbursement but they told that they have not received the documents. I had given them the the postal department's delivery report every time but every time your representatives mr. Prashant, mr. Mallikarjun, mr. Ishwar and others are continuously saying that we will let you no. But no response upto now.
                So i am requesting you to solve my problem immediately otherwise i have to go through legal way for my reimbursement.

                  I had been through an ovarian cystectomy at a top hospital, once i got discharged submitted all the documents related to the same immediately, after 2-3 months of me following up with them i get a response saying its fertility issue. I had a claim of just 35, 000. My doctor was surprised how could they say its fertility issue? Please help me to pass on this message not to go for family health plan insurance (Fhpl). It’s completely fake, doesn’t respond to calls or mails even after we paying for the insurance every month. No one should suffer like this.

                    Mediclaim bill submitted on 11/4/2018 for domiciliary treatment of arthritis/thyroad and hypertention which require prolonged treatment on continuous basis for unlimited period till death.in this respect the original prescriptions are very essential and no doctor will entertain without original prescriptions. Hence, finding no other means i had verified with the original and submitted to the fhplltd for reimbursement of the bill but the bills are pending for a long time for want of original prescriptions. I am a retired person and had served more than 34 years of banking service yet they do not allow and thinking it is a false disease. Hence, it s remedy?

                      I am r. Amirthabalan, claim for my wife delivery expenses. Treatment taken in srmc porur, chennai. She delivered twin girl babies, but in hospital they threatening for life to my wife about she is sick need to do dialysis. So we discharge early from hospital, they given the report as discharge against medical advice.

                      Then i am applied for insurance with in my hand all original medical bills and receipts. Now the claim is pending for consolidate documents. I am checked with hospital also, they not produce like that.

                      So please help and do the needful.

                        I am policy holder (1023942c06) of bharti axa which tpa is fhpl. On 20 feb 2018 i take patient (My daughter) which had only cough problem in fhpl network (Metro hospital, palam vihar, gurgaon). Hospital take my medical e-card and 10000 security fees and admit her in hospital. But now after treatment and time of discharge fhpl is denied for cashless claim... Fhpl had just time pass for 58 hours on telephonic discussion about documentation requirement from hospital. Hospital and me provide all documents even fhpl executive visited in hospital two times and collect all information. But at the time of discharging fhpl denied for claim. And give excuse that this treatment is not cover in policy. So i want to know that if this treatment is not covered in policy. Then why hospital and fhpl start treatment. I think it is a scam between tpa and hospital which are making fool to public and earning money.

                        Public should know that hospital make bill of 25000 in only 2 days for treatment for cough. And now hospital is not returning my security deposit 10000 even asking for 25000... So request you to please help me to recover this claim and be aware this type of company (Fhpl).. Thanks

                          Fhpl rejected my account claim saying that i have been submitted duplicate copies of invoices for my medical bills. They are behaving like i have been submitted fake documents. To get a confirmation for them i have contacted apollo where i took medicines and asked them for the original copy of invoice. They have stamped and signed on those medical bills. Even though fhpl rejected my claim. Such a pathetic behavior from these people. I need justice for my claim.

                            I have policy for rs.200000.00 from 01.05.2016 to 31.05.2017. I fall ill while i was at bangalore and mysuru. I received treatment from apollo hospital through o. P. D. And incurred an expense of rs.9019.75p (Rupees nine thousand nineteen and seventy five paise only. I submitted all documents along with the claim form filled in through my pension receiving state bank of india branch, badasankha, puri, odisha with due recommondation from the branch manager. Since than i contacted the insurance company through toll free no.[protected] to fhpl. They replied that my claim has been rejected for reason unknown to me.                                 Claim no.1237567 & 1258283.
                                This claim is pending since November 2016.Shame on FHPL.
                                Sending repeated same mails again and again without taking any information from fhpl customer care.There is no communication between fhpl document team & customer care.
                                As per discussion with customer care executive i sent all documents to fhpl.Few documents which fhpl required that documents already taken by fhpl representative from doctor who came for documents verification and same documents doctor is not giving again and again if these documents not reached to you then this is not my mistake for that you need to ask to your representative who came for documents verification may be the chances he has missed documents somewhere else.
                                FHPL is third class company.
                                Kindly take strict action against fhpl.
                                If your claims have been assigned with above ref nos, insurer has to decide the claim accepted or rejected within 30 days from dt of claim, if their is any delay insurer has to infm you for further documents requirement. Pl you may insist the insurer to settle the claims in writing, if the claim is rejected, insurer has to inform you the specific reason in writing or if no response is received after or within 30 days of your request, you may approach insurance ombudsman for your grievance within 12 months from dt of claim rejection or no response.

                                  I am an SBI retired employee, my P F Index no. is 248592 and Policy No. is [protected]P[protected]. Please refer my Claim Id no. 1287999 which was paid for Rs.2, 060/- against bill amount Rs.3, 914/- without showing any reason. On making queries 3 times, the dealing official seated at SBI, Zonal Office, Jeevan Sudha Bldg. Kolkata showed me the file containing the disputed bill. It was observed that the two staple pins were removed from the bill and four money receipts/cash memos were missing. The official argued that those documents were not attached. I have shown the photo-copies of the entire bill where copies were available but not given any importance. If this was so that I did not attach the documents though written in the bill, this should have been pointed out to me before removing the staple pins and before reducing the amount by Rs.1, 854/-, which was not done. The attending official continued to argue that they have no fault and hence no liability.
                                  Can you please tell me who is responsible for removing the staple pins without matching the documents at your office not advising the insured beforehand?

                                  May I seek proper judgement and get payment of the left out amount of Rs.1, 854/- ?
                                  I can submit photo-copies of the missing documents if necessary and called for.

                                  I solicit your kind co-operation in the matter.

                                    I admit my wife in muskan nurshing home @ moradabad in 23 january 2017. doctor is writing medicines from outside and he is saying cough is not covered in delivering time. i have fhpl also this hospital already take 42000 rs by your side. its totally fraud. please handil this. uhid is nic.16333222. all staff of hospital is wanted money. also behavior of all the hospital staff is not satisfactory.

                                        My dad is covered under a scheme in the fhpl. He was supposed to be discharged on he 17th december, 2016. But because of your pathetic customer service he has to stay at the hospital for longer.

                                        From morning 11 o clock i've been trying to get the initial approval from your concerned department and the same is still pending.

                                        So the billing approval is pending as well.

                                        We have always pond the premium on time and in time of need your company finds every possible way to trouble the clients.

                                        I'm very very disappointed in your service. I regret taking an insurance under your name.
