Dear sir, I have one policy of SBI Life insurance scheme through State Bank of India Branch-.No.001428, Amlai Paper Mill Po Distt Shahdol Madhya Pradesh 484117. This Policy premium deducted by My SBI Bank SB Account No: [protected]. for continuous from 2008 to every year Premium Amount Rs.524/-Direct Deducted. But i have not received the Policy Number, Policy Bond and Premium Receipt till today. so i request you to send it as soon as possible to my address as well as to my email... My Add.
