Unique Identification Authority Of India [UIDAI] — Aadhar card always goes to manual check status and gets rejected

Hi,

I have applied for adhaar card for nearly three times.
Every time it is going to manual check status and gets rejected.
I submitted ration card and pan card for proofs.
We have filed complaints by calling 1947 also but they only say that "we have forwarded your complaint to the concerned department, call us after 15 days".

I also went and asked the enrollment center. But they also don't know where things are going wrong.

The following gives the details of the latest application for my adhaar card which is again in the manual check status:

Please let me know what has to be done so that my adhaar card gets generated successfully.
