Vaccination — Non receipt of Vaccination certificate

Greetings,

We reproduce below a mail sent to Ms Poulami Sutra Dhar yesterday but we have not received a reply till date.

We wish to point out that our maid, Ms SWAPNA SARDAR, had received her 1st Covishield on 11th April'21 at your Fortis Hospital, Kolkata. Giving below are details of Ms SWAPNA SARDAR :

We would like to point out that she had till date NOT received her certificate.

I am enclosing herewith the following annexure as a proof that she had received her 1st vaccination on 11th April'21.

ANNEXURE 1 --SCAN COPY OF FORTIS COVID 19 VACCINATION CARD.
ANNEXURE 2 -- SCAN COPY OF COVID 19 VACCINATION SLIP.
ANNEXURE 3 -- SCAN COPY OF FORTIS CASH RECEIPT.
ANNEXURE 4 & 5 --SCAN COPY OF AADHAR CARD.

We would request you to kindly look into this & update your records so that she does not have a problem in getting her 1st Certificate, 2nd dose Vaccination & the final certificate . Due date is 4th July'21.
