Dear sir, i have availed a life insurance policy through my bank (Citibank, m g road, bangalore) from the above mentioned life insurance company in the month of december 2011. Below are the details of the policy: name of the plan: sarve shakti suraksha policy number: [protected] policy holder name: sowmya mohan, master policy holder: citi bank (Processed through my salary account). I decided to surrender my policy in the first week of october 2018. I sent the requested documents through courier. Courier details: bluedart airway, bill number : [protected], delivered on 4th october to the below address: Mr. K suresh Tata consultancy services insurance operations Tek towers, 7th floor, 11, rajiv gandhi salai Old mahabalipuram road, Okkiam thoraipakkam Chennai - 600097 I have not yet received any updates on this and it has been 15 days.
