Dear Sir/Madam,
I am to inform that I am pursuing my education in Pune, Maharastra since 2008. Suddenly, I lost me Corporation Bank and SBI, ATM-cum-Debit card on 05.01.2013 somewhere in Corporation Bank , Hadapsar Branch,Pune while carrying out some transaction in the Corporation Bank.
I request you to block my ATM-Cum-Debit Card transactions with immediate effect. The Account details of both the banks as follows...

my name is chitta subrahmanyam. my atm -cum-debit card no.[protected] valid from oct 2008. i forgot my pin no. hence it is requested to mail me fresh pin no. as early as possiable. i have already submitted my application to the concerned bank on 10.10.2009 and asked them to send my new pin no. to me by registered post..the copy of the application is also attached.kindly do the needful.

I used my above ATM card for purchase at M/S. Havmor
Restaurants, Ahemdabad, on 17/10/08 for Rs. 900/= (Rupees. NINE
HUNDRED only). I gone through my account statement and found double
time debited for Rs 900/= each. I contacted the concerned sales point
and their banks statement is showing...
