But this month they told me you get medicine from any medical store & submit the bill, it means they could not provide the medicine. Subsequently, on dated[protected], I have submitted a Medical Bill amounting Rs. 39957.04 (Rupees Thirty Nine Thousand nine Hundred fifty Seven only) with all the required documents to Incharge (Insurance Medical Officer), ESI Hospital, Mayur Vihar Phase-1, Delhi. At the time of submitting the bill, I was told that I will get the payment within maximum 2 months period. However, even after almost four months and frequent persuasions, I didn't receive my payment till date. I went two times to check my pregnant wife in ESI hospital but staff of ESI (Phagwara) did not treat us properly. They always say that lab machinery is faulty. I have submited all medical bills alongwith original copy of bills on approximate february 2014 month but till date i did not received any amount regarding this account.so please do the needful.

NOTE: Sir one thing i want to know from you that our contribution deduct from gross salary but at the time of reimbursement it calculates on basic salary why???? Please disclose me the what fact behind the story

I m Neha Verma i am private employee, I was on medical leave and was hospitalized for a week due to viral fever and infection doctors said it was johnson syndrome symptoms and had suspected it as mijeles, but when i went for my medical claim the doctors refused to file the case as they said it is not considered under ESIC medical claim and is not any emergency or accidental case, But according to ESIC norms any medical need or hospitalisation for atleast 24 hrs is under medical claim and will be Reimbursed once the claim is filed. So, If it is not considered as ESIC covered claim so why companies are cutting down our salary in the name of government else all the amount has to be refund in such case. Please take a strict action on this as it will help many other sufferers of the same.

As per doctor consult we go to room no 10 for insulation at 2:30 pm. But There was a nurse get miss behavior with us.weare requesting to you say that please take the Early action on this matter. Dear Sir/Madam, This is to your notice that in the dungarpur esic dispensary no permanent doctor sitting. Earlier, Dr Jignesh sitting in the sort time period but frequently absents from their duty or leave from duty. I have gone to esic for refferel purpose no doctor are sitting according to esic timming. Therefore, your goodself are request to depute a permanent doctor in the esic hospital so that employees can be take esic benefits.

My name is Krunal Savaliya. My insurance number is - [protected]. I met with an accident on 28th Jan 2014 while I was about to reach office as routine.There wasn't any external injury but I was not able to stand on my right leg due to internal injury. At that time I was taken to V S General Hospital.My x-ray report was normal so doctor suggested to do MRI report. In MRI report doctor found knee injury(ACL tear) and told that I do not need to go through any operation and suggested to take bed rest for at least for a month. They have given rest certificate also for the same. After few days back I visited ESIC Model Hospital(Bapunagar) on 3rd Feb to get temporary disability benefit. I shown them x-ray and MRI reports with medical rest certificate. Doctor checked me and approved rest for a month but ESIC personnel at DO16 insisted to give me rest certificate on the basis of ESIC doctor report card and told to get it approved from medical referee at Dariyapur.My wife and me went to Dariyapur and took approval of medical referee at dariyapur and submitted the same at DO16. But still DO 16 personnel gave me rest certificate for a week from 3rd Feb and told to come back after a week for extension of leave. My concern is that why I can not get temporary disability benefit from 28th Jan the date which I met with an accident. Government hospital certificate is no more valid for ESIC? If ESIC doctor is approving leave on the basis of the reports dated 28th Jan, then why they can not give me rest certificate from 28th Jan. Moreover while I visited ESIC - Paldi branch to submit documents to pass leave for a week thay told that accident report is not submitted by my company. I checked with my company and tried to resubmit accident report online but it is showing that accident report is already submitted for this person with reference number 277109. Also ESIC personnel told to get few more documents from ESIC Dariyapur branch. While my brother visited that branch Mr Gandhi (Medical referee) denied to give that documents stating that he had already given the same. Whereas in actual he hasn't give us any other document except RM 5 card while I and my wife visited the branch personally. I am not getting proper response from any branches. Every time they people ask to arrange one new document from another branch and while we ask that branch we get poor answer.Sometimes they misbehave also with us and tell to bring patient to ESIC branch and then only we give all certificates. I have also sent complaint mail to respective RO office as well as central contact person mail id but not getting any response from them also. Pls help

I have allready solved queries many time.

ESIC Slogan is "IP - VIP" but I haven't received the said rebate amount till date, So I request to you to do the needful as early as possible.

In the year 2002, my brother S.Abdul Nazeer, s/o H. Md. Sabjan, residing at Papanaidupet met with an accident to his leg in a factory near Renigunta. He was brought to Government Hospital at Tirupati. He has undergone treatment for his leg and his leg was damaged and unable to work further. After the above accident he became deaf and dumb. He was provided with ESI pension for few years. After that it was stopped at Tirupati ESI office. The particulars of his employement details he lost his house transfer. When he approached Tirupati ESI office they refuse to pay his monthly ESI pension. Because of his disability he is unable to hear and get the thing rectify for continuation of his ESI pension. Hitherto I was in Hyderabad and retired from Government service came to know now. Will the ESI official help us in restoring the ESI pension to my brother who was taking pension at Tirupati. My name is V.Nelson kumar Am the employe of Johnson lifts pvt ltd .i apply for esic, when i was went office and show my esic form they said your esic no:[protected] not updated, am return back to office and check every thing and updated my esic form, and went to the esic office, office staff said server not working you come by next day, again we came next day, esic office staff said photographer not in office today we are not allowed for photo and everything, you come by next day, on next day is Saturday&Sunday two days office not working because office leaves, when the office staff working properly how we completed our works, am educated person to send this mail what about the ordinary people problems, who can slow that problem, see how many times we can visit the esic office, my work is simple and it is time to tack 15minits, esic office staff taken 3days&Saturday and Sunday 2days total 6days, tack action this issue, why am send this mail i know some other person political leaders am completed my work, what about other peoples, that's why am send this mail.am not thing about my self i think about other people.give your feedback in this regard immediately

Dear Sir, With due respect of your reply I have to inform you that from my receipt no 18513 dated 24/06/2013 to till date there is no positive reply to get my smart card ([protected]) from ESIC LO SALTLAKE. Please inform me how many years I have to wait for a simple smart card issue??? We called several times to saltlake ESIC LO but no positive reply from their end. I visited several times and remind for the same but no fruitful results come till date.. Please inform me that, how many years I have to wait for a simple issue. If any positivity of this regards please inform me as early as possible otherwise I have to go consumer forum, TV reporters to lodge complaints against your service, because I cannot utilize ESIC facility but pay for the same, I think that is one type of wastage of money and for this regards I have to go for a legal step against your service grievance. This is very serious matter ESI is not a free service they are taking some amount from our salary so they need to give some concern about patients, they are very rude and irritating faces in this dispensary, if we ask any doubts also they are not what to answer and very rude we not having time. And also dispensary is very small and that is old house so this want to change. Please look into this matter and solve this ASAP.

Dear Sir / Madam, This is to inform you that, i have attend the schedule of Photo capturing for New ID card on dated 07.05.2010 . But yet to get my card, several times i have given complaint to the local office but till not received the same . A person of the department has advised me to give an application by mentioning that I have been misplaced my card and please re issue the same, I followed the same process and applied on 12.04.2013 for new card with a fine of rs.50/- .But till not received the Card . In Silchar, Assam there is a ESIC branch outlet but no dispensary & doctors. As per the Outlet incharge all patient have to go for Guwahati for any minor or major treatment or Governemnt hospital. Major treatment may not matter. But for all consumers, it is not possible to go to Guwahati as the distance is 12 hours journey through a another state, Meghalaya ( Distance is about 400 km ). Also there is matter of stay in Guwahati also. So requesting the concern person for a dispensary or doctors who can treatment the patient.

on 14 August at 23.45pm i fall down from my bike in such accident below my right eye part is injured . due to heavy blading i am unconscious some person admitted me in nearest hospital . after sergary in below my right eye 4 statiches . then after that hospital given bill of rs 4500 to me that bill i have submitted to esic hospital but esic docter in AMRAVATI MR SHEKHAVAT denied such bill he is said that why u r not addmited to our esic hospital so pls help me on repayment of my medical bill my contact no. is [protected] and all juniour and seniar doctor rufuse to atten me
