Respected Sir/Madam,
We wish to You inform that, I Mannaru Swamy Kothwala and this is my ESI IP Number-[protected]. By held on 2009th year, I claim for my medical equipment (like that, speds) bills (with 2, NOTE: In our Tirupathi ESI Hospital of claim/reimbursement register note down the Sl. No. 205-38th page-550-00/- (Five hundred and fifty rupees are only) and another one bill of Sl. No. 281, 114th page- 416-00/- (Four hundred and sixteen rupees are only).

I submit the above equipment bills are attached and claim/reimbursement required to all documents. It's will be forwarded to The Joint Director Medical Services of Cuddapah office and me number of times are go to Tirupathi ESI Hospital. But, claims dept. off concerned employees says to me "WE PLEASE WAIT & INFORMED BY YOUR'S COMING THE CHEQUE AFTER"... I claim the bills 2009 & 2010 of the year. But, up to this (till date:[protected]as usual same dialogue will be repeated. We think it's you once to how many days are completed...?

By held on 25th January-2014, as usual go to Tirupathi ESI claims Dept., That's department door closed (its stand or waiting for one hour more than with me). I ask to dispensary employees and we says to see and searching you man and I went to hospital officials ask to I don't know that details. This is the responding to IP HOLDERS (WE ARE ALWAYS ADVERTISE TO IP IS VIP). Those who have not responding with me. I think, this type of bill are pending to one of me and more than in Tirupathi ex specially.

I am really sorry to says about that, these are by intimated to you for me acknowledgement only. We once again, request you to see this mail of attaching file and give me my claim/reimbursement bills of status report and why to delay for that totally (416-00 +[protected]/- (Nine hundred and sixty six rupees are only). After, I through RTI to Tirupathi, Cudapah and Secundrabad DIMS offices. But, no use for me...,

I wants to my total reimbursement amount will be settle at quickly as soon as possible.

It is very difficult to avail services of Gurgaon ESIC hospital. The security staff is very rude by nature. They use profane language with consumers. In OPD they make 10 to 15 people stand in one go. The relatives of security or ESIC staff does not need to stand in QUEUE. They do not care about the severity regarding the illness the patient is suffering from. Simple blood reports which takes 2 to 3 hours to publish come after 3 days. The sample that ESIC sends to another lab outside of ESIC comes after 7 days. The big thing consumer has to go to dispensary for medicines. Most of medicines are not available in dispensary. The dispensary staff says to purchase medicines from outside and submit the bill. Again there is only one day that is Wednesday to submit the bill. The paper work is so pathetic and overall it is very difficult for a patient to wander here and there. Most of ESIC staff do not think necessary to help the consumer rather than busy in their smart phones or gossiping.

Today we went for treatment today but they said the form 37 is not in force. i received the form 37 from my office on 09.08.2014.i was asked to go to kk nagar tomorrow to have treatment.i would be very much greatful if know where the problem is.

We are Empirical Engineering Enterprises, 9-Lake Road, Ramakrishna Pally, Benachity, Durgapur-13 . Our many of Employees are not received any Bio-Metric Card for long time. We send several time letter of our nearest brunch but they do not take any action. If any accident accrued in inside of plant in favour of our company, we should facing a big problem and we submitting their contribution properly deposited in every month. But our needy workers not get any facility for ESIC department.

I am informing that I have not yet received my Smart Card against my Insurance no.[protected] (Goutam Sarkar) At first our picture captured on February, 2013.But due to not receive the smart card I had applied for duplicate Smart card on 09.09.2013(Book No.CC 1500, receipt no.139956) against Rs.50.00 at ESI Dum Dum branch. but still today I have not received any smart card. Please enlighten me in this matter.

SirMadam I am an insured Woman under ESIC and applied for maternity benefit near our Branch Office situated in karunagappally, kollam (dist), kerala. My date of Appointment and joined in service was on 09/10/2013. My Confinement date was 14/06/2014 and date of discharge from hospital was 20/06/2014. I gave certificate of confinement to Branch Office on 24/07/2014.When I gave application they said that I am not eligible to claim Maternity benefit because to claim maternity benefit one should complete minimum Nine months service .So I kindly request to solve this grievance regarding the period of service that an employee should complete to claim Maternity benefit from ESIC. This is Aravind from chennai my father was worked at leather factory (ECTC Periyankuppam), Two years ago he was hospitalized due to Hypertension we have admitted him at Vellore CMC Hospital. we have spend around a Lakh rupee, after that we approached nearby ESI office to claim the amount. And they said that they are eligible to claim a amount below 50, 000 only so that they forwarded the request to Salem office dated on 05/08/2013 still now we didn't get any single response from any office about the ESI amount. My father is Kidney patient whose above 55yrs cant roam to office daily to ask about the details but the officers at ambur asking again and again to send the details to branch. This is the third mail im sending to your mails id's there is not a single response from your side. if it happens again i will sure file a case in consumer court. Hi, This is Nijanthan from Chennai and One of my employee was approached to ESI to take treatment and since he is having TIC and not original ID. But the person in ESIC office said your company already registered but why you not having permanent ID and you are not eligible to get treatment and all. So finally the big argument comes to an end after giving 250Rs for the Treatment. The officer simply get the money from our employee and allowed for Treatment. ESI is provided for getting Health facility and it is based for less salried people but how the ESIC employees are treating the people who visiting for wellness. i am mr N.radha krishnan from chennai.my i.p.no is[protected]. as i contacted a local dipensary in harbour area for a diabetics medicine . but they are saying that medicines are not available and they also insisting us to purchase from outside medical shop. if this made to happen why i need this facility. i requestd you to take proper action against this and make medicines to available as soon as possible thank you.

I am a person from defence, Indian Navy - INS Garuda. My wife is a Pharmacist and she is working in Lakeshore Hospital, Kochi. Her ESI account is at ernakulam office, near North Railway station. My wife, Nisha had taken leaves from[protected] to[protected] as she was having Bronchitis and applied for ESI leave. I was the one who collected the ESI letter from ESI office to the hospital as well as to the ESI office. On[protected] we went to ESI hospital to collect fitness certificate but the printer was not functional hence we had given a slip stating that she is fit from[protected]. Using the same slip, she joined for work. Later on[protected], the printer was repaired and we got the fitness certificate. Again i visited the ESI hospital to submit the fitness certificate with the copy of the slip. Now they want the doctor to write on the certificate that, she was fit from[protected]. See already we are presenting the document that the Dr has given, still what is the point in writing the same again. Getting that done is not tough, but to travel that distance to submit is the issue. Since i have an ESI card, i know the rules and regulations very well. But the officer in ESI says that the rules in Indian Navy and that of their are different. If there was an online way to submit the documents would be an option? A person requesting for ESI leave, who is not well and fit - How can they submit the documents to an office which is too far say 60Km? This is not fair, to take money from employees and denying the claim. If its to deny, why this scheme is created. Or is it another Govt policy to loot money from people and give it some employees. Kindly create an online media to submit the scanned copy o[censored]nfit and fit certificate for the employees. We the people dont demand respect, but please dont pity us. Its with our money you people in the ESI organization is living!!!

I am an esic member & my IP No. is [protected]. I would like to place a complaint against esic dispensary B-block sector -57 NOIDA.incharge Mr. Gupta & other 2 doctors.I don’t know the name of other 2 doctors because it was my very first visit to that dispensary. It was very bad experience my life. I am an asthma patient .I was at esic dispensary on 29May2014. Doctors of that dispensary misbehaved &tried to beat me up. They told us to go somewhere else. They also made fun of my illness. One of the doctors asked me to pay commission if I take costly inhaler of asthma. He abused me when I refused to pay. So I am asking you to take strict action against those doctors.I am very upset. Please give me justice.

Procedure Taking appointment consulting doctor medicine slip+Eligibility certificate+Form-37+Esi Card every month submitting in Dispensary but the dispensary special medicine issue in charge persons not issuing medicine on time
Please take neccesary action to cooperate the patients to receive the medicine on Time
(Same MG Dose to be supply overdose Medicine can cause patients inconvenience)

my name is shoba my collleague miss. s.vijaylakshmi visited your esic dispensary karaiyan chavadi, for medical assistance. they are asking money for even registering her esic account, for everything they are asking money for eg. registering the account, issuing the leave giving medicine etc., already the companies is deducting esic for every employees then why they are asking money for medical treatment, it is their duty to give medical assistance. please take this matter seriously and please give us favourable reply.Take immediate action. so that in future nobody will suffer.

I Ms. Rasshmi Shinde having an ESIC Insurance No: [protected] and UHID No: MH01.[protected]. I have claim medical bills of my father who is dependent on me, for settlement in the month of April 2013 at Kamgar Hospital, Wagle Estate. The file no. given by the concern officer is R-5. The settlement of the same has not been done till date. Whenever ask to the officer about the file, they say come after 1 month or 2-3 months. I have also done the mail to [protected]@esic.nic.in many times. In December 2013 I had received a mail from Mr Sunil Kr Singh SSO, ICT Division informing me that my file has been forwarded for information and necessary action. But till date there is no improvement at all. I am taking the follow up regurarly and now I am really fed up of the service given to me. Details where the file has been submitted are as follows. Today I visited first time in ESIC hospital Bari Brahmana Jammu. As my finger got fractured and I need treatment as soon as possible. But after getting tickets from counter I got X-ray machine was not working. And the concern ortho Doctor is not available for 2-3 hours. And I am screaming on waiting table.
