Sir, My ESIC IP NO :[protected], Name Jithesh. I have a complaint regarding reimbursement.My ESI dispensary is at Kozhikode, Kerala.I am hospitalized due to vomiting and dehydration in emergency dept on 15/06/2015 at 8pm and admitted 3 days.Next day (16/06/15) i reported the ESI office and dispensary, they told to fill and submit the Essentially Certificate and all bill. Asper the instruction after my discharge i went the ESI dispensary and submit the emergency admission certificate, all bills and the form, but the Doctor rejected my claim and told me to complaint ESI Director. Now i am not well and not able to do my job. I paid around Rs.9000 in hospital, i am a poor family member also. Because i kindly request you to do the needful to grant me the amount. I have not recieved my ESIC smart card which was made in year of 2014 . But card still not recived. when i ask to ESIC staff they replied card has been dispatched from our side.

Respected Sir/Mam, I am writing this from Calicut, Kerala, Here we had some number of ESI approved hospitals but nowadays the number of hospitals getting decreased as they are not getting payment from government, Due to this problem we people are not feeling any security consulting at those hospitals, Because for a major surgery and all we people under ESI cant afford the bills, So kindly request you to ensure that we will get secured service at ESI approved hospitals as soon as possible.

Each Ip number shows duplicate no. whenever I enter the no.of days under the monthly contribution. We are only with 4 employees and it shows error like the above. When I contacted the local esi office at pondicherry, they asked to contact you over email and mention the problem as there are no duplicates available on verification. respected sir/madam, we the kanyakumari district people not having the esic hospital here so we suffer a lot if any serious condition arises for the patient then they are supposed to go to triunelveli esic hospital only and not having any proper treatment for infertility . so please consider this .and we want esic hospital in kanyakumari district.every employee is paying certain amount from their salary .but no response.

Recently my father injured form brain tumour and got treatment in super speciality hospital on paid basis, after discharge for reimbursement of the many i had submit the all documents bill to the local ESI office for the amount of close to 2, 10, 000/- after 3 months the amount has came but which less than my original bill that is 53, 100/-, even the officer din't show me the cheque to me. so kindly please help is this amount is right.

Hi, I was an employee of VDA Infosolution Pvt. Ltd, Address - 76 Shakuntala, 5th Road Khar (West), Mumbai 400052. My Work Location is in Kolkata. i worked there from 1st January 2012 to 31st January 2015. During my teneure They made the deduction for ESIC from the very First Month till the last month. But they never provided any ESIC number, neither it was there in payslip. I mailed them sevarel time but not get any response. Please look into this as the whole money has gone and i never got any benifit. And they did the same for many other user also.

Dear sir I rakesh moundekar having esic no [protected]. I came for medicine for fever at model colony nagpur despensary. There is time for despensary is 4 to 6 but doctor yet not came till 5.30 pm I came from for medicine during office time. It is regular process of doctor to come let. Dear sir, I am Sukanta Parida went to ESI Hospital Indiranagar for my wife regular check up there was doctor given the report thyroid problem, You go to rajajinagar ESI Hospital you have to scanning report collect and come back, so we are rajajinagar hospital register the compliant there was told wait some time and then came told me come and next week it is happen three time return back to home. Most Respected Sir, Sub: Request to Grant ESI Maternity Salary Benefit-Reg:- It is to kindly intimate you that I am working as Staff Nurse in a private hospital in Bangalore since August 18 2014 to till date. My ESI number is : [protected] .My delivery was on 21/04/2015. I went to ESI office in Bangalore on 05/05/2015 to give the documents for availing my maternity salary benefit. But unfortunately they informed me that I would not get the salary benefit since I have not completed 9 months in the company. Only a few fays left to complete 9 months. Due to this reason I will lose my salary for 3 months. I request your good self to grant me at least the amount proportionate to my service in the company. Since my salary is below Rs.15000/-, my company will not pay any salary to me in the maternity leave period. My family is totally dependent on me. Dear Concerned, My Name is Niranjan Kumar Srivastva S/o Sh Ishwar saran Srivastva and my insurance # [protected]. I got my Father's Eye operated from Maheshwary Eye Hospital, Amritsar referred by ESI Hospital, Amritsar. The operation was done on Feb 26, 2014 and bill was submitted on Apr 7, 2014 for the Rs 11400/- approx. But till date i have not received the bill amount back. Whenever i ask the ESI personnel, they always reply very casually, that it'll come. Please into the matter as one year is passed, and not any proper response is provided by ESI officials

hi sir plz accept both Written like computer printed and with photo copy with stamp and Pahachan card note: They are see in hospital very bad plz give proper traing to ESI Staff and accept all Pahacahan card and Esi printed with photo and stamp plz give permission for all the patient we have snapped for Pahachan card but its not give they are not accept paper print and photo with stamp plz see and give necessary action ThanQ

I have admitted in private hospital in Vapi (Gujarat) from 15/04/2015 to 19/04/2015 on my own expenditure. I want to reimburse the coast of treatment which include hospital charge and medicine of around Rs 9000.00. I want to know the procedure for reimburse, I have contacted ESIC Vapi branch and they denied to give any information on this regard.

On 23rd of April 2015, due to Internet server mischief the doctor in the ESIC wrote prescription on paper only(unlike their regular prescription online) hence, the store refused to dispense medicine. On refusal to medicine we returned to the doctor who refused to help and sent us back to the store who refused to provide medicine. This went on numerous time and on enquiry our only hope of a good health care of our kid went off when we found of the unavailability of any supervisor impression. After a number of refusal and 6 hours of our time in bad health of our kid we came back home.

There the ESI doctor Mr Naresh is not approving for ESI and he is telling all the stories that we did not get claim.We have argued with him on this regard and no issue and he also tear off all the documents submitted and he is telling that go and tell any body Iwont care any body.Hence Iam putting this in front of you.Kindly help me to get approval for ESI.
