ICICI Bank — Return Identity Proof Documents

This to inform you that Mr. Manish Kumar on behalf of ICICI bank had offered me a platinum chip credit card and came to me on 17/06/15 for filling up the application form having no. N[protected] and collected the photocopy of my various ID proof viz: Pan card, Adhar Card, photograph, voter id card. Till yet no credit card has been received. As per the telephonic conversation that I had with Mr. Manish, he has left the job. He has provided me a no. of another sales executive in branch. On contact with her, I came to know that she also left the job. It is a very serious issue and irresponsible work from the branch. I want my documents back as mentioned above as early as possible.
