I had taken a postal life insurance policy no-APS/NM/124528-L date of proposal-22 Oct 1994 for Rs.40, 000/- in the name of PRAMOD KUMAR when I was in central government service Indian Army. The premium of above said policy of Rs 206/- was directly deducted from my payment during service, But after got retired from service in the month of Jun 2011.But before retirement all premium deposit and stop subscription but not getting maturity amount .Date of maturity - 30 Nov 2011 after retirement transfer application, last QSA, and subscription stop QSA, also BOND photo copy registerd to your office but not any information also reply getting me . The post office near my home is JAMUI post office, Dist- JAMUI. So kindly request sir plese reference ask me through contact no or emai.

2. I was AC(UT) NEERAJ KUMAR JASRAPURIA having service no-8612868 in 405 AIR FORCE STN, BELGAUM when I have opened a PLI through your organization bearing proposal No – 784932/102/EA/40 & amount of Rs. 435/- per month is deducted from salary slip from the month of JUNE 2006 and being continued.
2. Even after 7 yrs, subscription of premium of concerned policy certificate alongwith policy bond has not been received yet.
3. It is therefore, requested that the original Bond certificate of PLI proposal No. 784932 in r/o neeraj kumar jasrapuria may please be sent to my under mentioned address at the earliest for which I shall remain grateful to you and your department otherwise please inform me further action by return of post.
