My Form had been Deposited my Provident Fund withdrawal form at wazirpur office on 16-June-2008. It has been now 90 days and have not received the same so far. Nobody picks up phone and don't know what to do. Kindly do the needful so as to receive my money from PF office.

I have submitted the PF form in Nehru Place on 24ht Sep.-2008 & that returned to my office reason for defult speling of my father's name after that i have resbmited at Wazirpur epf office before 2 months from till date.But since this not cleared. Most regretfully, I bring to your kind notice that my monthly pension has been ceased by your department since 1st January, 2008 without any reason.

On 20th March, 2009 I had come to your above mentioned office and submitted original Life Certificate from Punjab National Bank, Trinagar, Delhi-110035 as demanded by the Lady dealing Assistant holding the seat of releasing pension in your department and I was assured by her that the payment would be sent to Punjab National Bank, Tri Nagar, Delhi from 1-1-2008 in the month of May, 2009. But all in vain.

Despite of my repeated request and submitting fresh life certificate which bank had already sent in the month of November, 2007 my monthly pension has not been released so far. A copy of the Life Certificate which was handed over in original on[protected] in your office and a photo copy of the gate pass taken for entry in the office are being enclosed herewith for feeling the facts.

Please look into the matter and arrange to release monthly pension from 1-1-2008
