i applied my pan card and received also now i missed my card durung my traviling i have no photocopy i have only pan number. my number is AUJPM7071J. now what can i do please give me a suggestion.

I lost my pan card, I lodge a complaint in near by police station and got FIR, and I have a copy of my form 16 which indicates the pan card no and i have a copyof pass port for ID proof. Now I want to apply for duplicate pan card what are the procedures to apply for duplicate pan card? will you please suggest me.
