I would like to make a complaint regarding my lost mobile phone. I lost my mobile phone while bus which started from chingrighata 10.00 am on January 5, 2011. When I got down at science city,kolkata,WB at 10.15 am I found my cell phone missing. Please find the details of the mobile phone below. I lost my mobile phone while I am on the way to my Office at 14 N. S. Road, Kolkata - 700001 from my home t 199 C. R. Aveune Kolkata[protected].00 am on August 8, 2012. When I got down at Office 11.30 am I found my cell phone missing. Please find the details of the mobile phone below.

Model: Samsung 699 CDMA
Mobile Number: [protected] (Reliance)
IMEI Number: A000001D7E36D0
Kindly request you to block the mobile using the IMEI number mentioned above. I attached my Bill copy for your ready reference.
