With due respect and humble submissions I would like to inform you that I have appeared in SSC Combined Graduate Level Examination, 2011 (Tier II) at Guwahati in Railway Higher Secondary School centre on 04.09.2011. My Roll No. and Ticket No. of the said examination was [protected] and 5000504 respectively.
That thereafter I qualified the aforesaid examination and as a result a letter was forwarded to me to appear for the Data Entry Test for the post of “Tax Assistants” (Tier II) which was held on 26.11.2011. But I was not interested of the post “Tax Assistants” (Tier II) as because at that time I was serving as an ASM at Habaipur in the Grade Pay of Rs 4200/- (Rupees Four thousand two hundred) only, so I did not attend the test. The other reason is Railway authority would not allow me to join in lower grade pay, as because at that time the grade pay for the post of “Tax Assistants” (tier II) was Rs.2800/- (Rupees Two thousand eight hundred) only.
Thereafter again two call were dispatched to me on 30.12.2011 and 06.01.2012 but I did not attend the test due to the above cited reasons.
That thereafter surprisingly I got one “Show Cause” notice on 14.02.2012 from The Deputy Director, Staff Selection Commission, North Eastern Region, Guwahati, addressing to me vide F.No. SSCG-A-11013/08/2011Exam/Vol.II/187 stating inter-alia that I have resorted to malpractices in the said examination and within 7 days I have to reply to The Deputy Director, Staff Selection Commission, North Eastern Region, Guwahati as to why my candidature should not be cancelled and why I should not be debarred from the examinations conducted by Staff Selection Commission for a period of 5 Years.
That I was at home on leave at the time when the Show Cause notice reached at Habaipur Station. I got the Show Cause notice after 1 and half months. As soon as I got the Show Cause notice I was very shocked and immediately I went to the office of The Deputy Director, Staff Selection Commission, North Eastern Region, Guwahati, to meet with a reply letter but the staff of your office did not allow me to meet The deputy Director, Staff Selection Commission, North Eastern Region, Guwahati. I tried my level best to meet but failed. Then I handed over the reply letter of Show Cause notice to the office and returned to my station.
Thereafter I got a Memorandum on 06.06.2012 from The Deputy Director, Staff Selection Commission, North Eastern Region, Guwahati, stating therein that there is a prime facie evidence that I have resorted to unfair means in the examination held on 04.09.2011. So my candidature has been cancelled for the said examination and also debarred me from all the examinations or selections conducted by the commission with immediate effect.
I was very shocked after getting such type of memorandum as because I did no engage in any kind of malpractice during the examination in exam hall as alleged in Show Cause notice. The CBI also interrogated me on this issue on date 25.4.2013 but I successfully answered all the questions put to me by the CBI.
Dear Sir I am such a type of person who always dreams for a bright career.it is not possible on my part to adopt unfair means in examination hall. I have been preparing myself to appear in the competitive examination for the last 15 to 16 years and still I have ambition to clear the SSC Examination but due the injustice done, my future ambitions looks cloudy as I am very much disappointed against the procedure adopted by SSC as I am much innocent and no way connected with the alleged malpractice. So i request that i may be given a chance to defence myself of the allegations levelled on me as the allegations are false and vexatious and I am sure that I shall able to prove my innocence in this respect.
Therefore it is prayer before Your Honour to admit my petition and allow me to appear in SSC CGL 2014 and further be graciously pleased to considered my candidature for the said examination and the decisions taken by the SSC, NER, Guwahati to debarred me from all the examination or selections conducted by the commission with immediate affect after hearing my side for the natural justice and oblige.

There are three VOter card where the above change need to be affected for 3 family member Debasish, Pampa and Anjali Mukherjee. I am from Bally Constituency (169). My Family has moved to a new flat under same constituency but diffent road (Monomohan Mukherjee Road to Panchanantala road (Part 14), both under same ward No 3) and accordingly I submitted requisite form and relevent documents last november and attended the hearing date as informed to us. However no body can provide us information why this is not done. I feel as per the right to Know the information Act, I would suggest Election Commission to let us know why this has not done?

THIS IS REGARDING MY VOTER ID CARDS WHICH IS NOT RECEIVED TILL DATE. I HAD APPLIED THE SAME 4 YEARS BACK AND MY NAME ALSO EXIST IN THE VOTER LIST. This is Ankita, resident of ward no 12, ariadaha, under kamarhati municipality, Its been more than 4 months that I have applied for my voter ID card, and the officers from municipality have asked me to report about the availability of same after august, (applied on april, 2013) and now when I went there, my name is not there in the list. I am Avik Baheti from Khandwa, Madhya Pradesh. My complaint is about to voter Id card. I have submitted my all details 3 to 4 times for voter-Id card but can't get it correct voter Id. Now I have 3 voter Id card with wrong details In first they did mistake in my name instead of AVIK it is ANIL in card when i submitted to correct it then second time they did mistake in my date of birth .After this I again provide some document for correction in DOB so finally this time did mistake in my surname as well as my father name so it is not acceptable in passport office cause of wrong name.
Now this time I really afraid that if I’ll give my document and they'll correct my surname and will do some mistake anywhere else.

Now I am really very tired. I really don't know what I have to do. Whom I said

Above mentioned are the details as per the voter searching online system. I tried several times to inquire about the original voter ID card at district collector office but in vain. The most significant failure in this matter is the contact person mobile number who is BLO is this case Piyush Vyas who has provided the wrong mobile number. It's his due diligence that the voter card reaches the right person in time. On inquiring on the given mobile number some one else picks the calls and on visiting the BOOTH(Deepak school,nava nikol) they say there is no one responsible for our society. We don't know anything about the status of the original Voter Id Card. I afraid there may be some misuse of the same. Request you to please take necessary actions on this and update us asap. I called & mail so many time to SDM office or BLO Office but every time not get any response and given to assurance will send you soon only.

Sr No 192.In order to receive my voter id card we have been walking to respected office in

My voter card no. is NJK0063529 After application i didn't get my voter card. but later i have to get a duplicate copy of voter card by paying some amount as well as running at many offices. it has wrong birth date of me xx/xx/1988 but My actual D.O B is 08/10/1987.
